Social Services is the gateway to all SBSS programs and services, as well as other community resources.
SBSS' team of multilingual care managers work closely with older adults and their caregivers to assess their individual needs and preferences, and develop an individualized plan of care. Clients maintain long-term relationships with their care managers, who serve as advocates for them at each critical juncture of their lives and provide person-centered care management to fit their unique needs.
